Driver killed in early-morning head-on collision in west Phoenix

PHOENIX – One person was killed and two others were injured in a head-on collision in west Phoenix early Thursday, authorities said.
Officers responded to an injury collision near 67th Avenue and Osborn Road, between Indian School and Thomas roads, around 2:30 a.m., according to the Phoenix Police Department.
One of the drivers, identified later as 36-year-old Albert Cervantes, died of his injuries at the scene.
Two adult occupants of the other vehicle were hospitalized for unspecified injuries that aren’t considered life-threatening, police said.
What caused fatal head-on collision in Phoenix?
“The preliminary investigation indicates that the deceased victim’s vehicle was traveling southbound on 67th Avenue when, for unknown reasons, it entered the northbound lanes of traffic and collided with the other vehicle,” Sgt. Rob Scherer said in a media advisory.
No other details were made available. The investigation is ongoing.
